Enhancing Patient Safety Through Success Cause Analysis and Safety-II Approaches

In this session, experts from Stratis Health delivered an insightful discussion on patient safety methodologies in ambulatory surgery centers. They introduced an innovative approach that integrates Success Cause Analysis (SCA) with traditional Root Cause Analysis (RCA), offering a well-rounded strategy for quality management. We explored how examining both successful practices and adverse events can foster a culture of continuous improvement and resilience.

During the session, participants:

  • Gained an understanding of Safety-I, Safety-II, SCA, and RCA approaches, and their unique applications in patient safety.
  • Learned to identify and analyze factors contributing to both successful outcomes and adverse events in healthcare settings.
  • Discovered strategies to enhance staff engagement and foster a positive safety culture by recognizing and reinforcing successful practices.
  • Identified skills to implement proactive, continuous improvement strategies that focus on system resilience and adaptability in patient care.

Alona Jarmin, RN, BSN, MSN
Alona Jarmin is a Program Manager at Stratis Health. She began her healthcare career as a CNA and holds a master's degree in nursing from Walden University. She is pursuing a doctorate in nursing practice with a specialization in Psychiatric Mental Health Nurse Practitioner. Alona's nursing practice started with the Indian Health Service, serving the Turtle Mountain Band of Chippewa Indians, where she is a tribal member. Throughout her career, she has held various roles, including nurse manager and Quality Improvement Specialist, and has supported health system improvements in rural and urban settings across several states.

Dr. Jane Pederson, MD, MS
Dr. Jane Pederson is a geriatrician and healthcare quality expert serving as the Chief Medical Quality Officer at Stratis Health. She received her medical degree and completed her internal medicine residency at the University of Minnesota, also earning an MS in Health Services Research and Policy from the same institution. Dr. Pederson has extensive experience in clinical geriatrics and healthcare quality initiatives, with involvement in various professional organizations and national committees. Her approach to patient care emphasizes understanding patients' preferences to maximize their quality of life and well-being.
